Community Arts Engagement Subgranting Program: Design of Grant Data Collection Forms

The purpose of this request for proposals is to solicit a contractor to design the forms and methods for collecting data to support the performance measurement and program evaluation for an upcoming Creative Forces® Community Arts Engagement Subgranting Program intended to be launched by the National Endowment for the Arts in 2021. This nationwide program will provide grants to organizations offering arts opportunities to military and veteran-connected populations in nonclinical, community-based settings. Grantees will be required to address specific outcomes identified in the Creative Forces Community Engagement Logic Model and will have associated data collection requirements.

The overall intent of this contract is to produce a grant data collection plan (i.e., identifying relevant data elements/indicators collected from grant applicants and recipients at pre- and post-award phases) and associated data collection forms/instruments including, but not limited to, supplemental data form supporting the grant application process, a final grant report form, and survey instruments collecting program feedback and outcome-related data from individual program participants and grantee organizations to support the Creative Forces Community Arts Engagement Subgranting Program.
