Art Pharmacy Care Navigator – Job Description

Art Pharmacy is an innovative organization providing the means for healthcare providers to ‘prescribe’ arts and cultural experiences to clients for the prevention and reduction of various behavioral health issues. Job Summary
The Care Navigator is the main point of contact for Art Pharmacy clients. The Care Navigator builds strong relationships with referred clients in order to help clients engage in health-improving arts and culture experiences. Care Navigators are committed to reducing the clients’ barriers to the experiences by identifying obstacles with clients, helping them navigate transportation issues, and promoting relevant experiences to increase the chance of uptake.
The Care Navigator’s primary responsibility is ensuring organizational effectiveness by providing thoughtful delivery of Art Pharmacy day to day services. This position also contributes to the maintenance of arts & culture offerings database; development of relationships with strategic partners; evaluation and analysis of client experience; and programmatic strategies, policies, and practices.
Duties & Responsibilities
The Care Navigator will:
- Work in collaboration with the Art Pharmacy team to deliver high-quality, coordinated support to clients to access and engage with Arts & Culture experiences through the Art Pharmacy platform.
- Field inquiries from referred clients via phone, email, and text to connect clients with appropriate Arts & Culture experiences from the Art Pharmacy’s offerings,
- Book and coordinate logistics for client participation in selected Arts Engagements and provide relevant information/tickets to the client.
- Maintain up-to-date records of client activity and participation status
- Manage a caseload of clients from entry to Art Pharmacy (referral) through intake assessment, regular arts engagements and midway assessments, to end of prescription evaluation.
- Maintain open communication with Arts & Culture Partners to maintain up-to-date calendar offerings and relay attendee information as necessary.
- Participate in strategic relationship-building initiatives with Arts & Culture and Prescribing Partners
- Contribute to the development of Art Pharmacy services and participate in support, supervision and training as required.
- The role is a non-clinical role, however the Care Navigator will receive orientation and continued training to best be able to use social prescribing to empower people to take control of their health and wellbeing.
- Ensure compliance with all relevant policies and procedures in relation to health and safety, equal opportunities, data protection and confidentiality.
General Qualifications
- Excellent listening and communication skills (both spoken and written) – must be comfortable speaking with strangers on the phone and video chat, as well as via email and text.
- Excellent organization and record keeping skills
- Highly proficient with database management and use of Google Suite
- Takes initiative in finding solutions to problems and thinks creatively in the face of obstacles
- Demonstrates a practice of empathy, emotional resilience, and non-judgement
- Experience working with diverse populations (age, race/ethnicity, abilities, language, etc)
- Proactive in maintaining personal and professional learning and development and identify own training needs as they arise
- This is a remote position, though a familiarity with Stanford University and the San Francisco Bay Area, and ability to navigate these cultural communities, is required.
Compensation & Benefits
Salary: $5,000 per month
Health Stipend: $250 per month
Remote Work Stipend: $250 per month
